APPLICANT REQUESTS THAT:

His DD Form 214, Certificate of Release or Discharge from Active 
Duty, Block 11, Primary Specialty, be corrected to reflect his 
awarded five skill level.  



APPLICANT CONTENDS THAT:

His DD Form 214 does not reflect the award of his five skill 
level.  He completed all required upgrade training and was 
awarded a new Primary AFSC of 2A352 on 30 Nov 11.  

In support of his request, the applicant provides a copy of his 
DD Form 214.

The applicant’s complete submission, with attachments, is at 
Exhibit A.



STATEMENT OF FACTS:

The applicant initially entered the Regular Air Force on 29 Jan 
08.

On 30 Nov 11, the applicant’s commander signed an AF Form 2098, 
Classification/On-the-Job-Training Action awarding a new Primary 
AFSC of 2A352. 

On 28 Jan 12, the applicant was furnished an Honorable 
discharge, and was credited with 4 years of active service.

described in the memorandum prepared by the Air Force office of 
primary responsibility (OPR), which is included at Exhibits C.




AIR FORCE EVALUATION:

AFPC/DPSIC recommends denial, indicating there is no evidence of 
an error or injustice.  A review of the applicant’s official 
military record confirms award of the AFSC 2A352; however, there 
is no documentation to support he performed duty in that skill 
level.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to the 
applicant on 1 Oct 14 for review and comment within 30 days 
(Exhibit D).  As of this date, no response has been received by 
this office.



THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by 
existing law or regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence has been presented to 
demonstrate the existence of an injustice.  After a thorough 
review of the evidence of record and the applicant’s complete 
submission, we believe the applicant is the victim of an 
injustice.  While we note the comments of AFPC/DPSIC indicating 
that relief should be denied because they could not find any 
documentation to support the applicant performed duty in that 
skill level, we believe a preponderance of the evidence 
substantiates that corrective action is warranted.  In this 
respect, we note the applicant was awarded his five skill level 
on 30 November 2011 which supports the applicant’s contention 
that his records are in error.  Therefore, we recommend the 
applicant’s records be corrected as indicated below.  



THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air 
Force relating to the APPLICANT be corrected to show that on 28 
January 2012, his DD Form 214, Certificate of Release or 
Discharge from Active Duty, Item 11, Primary Specialty, be 
amended to include 2A352B, Integrated Avionics Systems 
Attack/Special Craftsman, CV-22, F-16, 3 Years and 10 Months.
